Certification and Recertification

• Examination Candidate Handbook - September 12 - 13 Examination Handbook not yet available.  This is the April 2008 Candidate Handbook for general information.

• Internship Forms - Internship Forms and information.

• Certification Renewal Application - Recertification is required every 3 years and is due December 31

• Certification Renewal Application with recertification requirements and listing of approved seminars for current renewal period

• Certified Doctor Referral Permissions Form - All doctors whose certification is current may be listed on the online referral site. Complete form and return to office.
